All participants for the [ACTIVITY NAME] are required to complete this form. The form must be complet-ed and returned to event organiser prior to the commencement of the event.

 

As a treat for everyone’s hard work over the last few months and to celebrate the company success, 372 Digital Pty Ltd has organised to take everyone to experience the [DESCRIPTION OF EVENT]. 372 Digital Pty Ltd will fund for all staff overnight accommodation at [NAME OF BUILDING] located at [ADDRESS], meals and return travel to and from the destination. It is on your own free will and at your own expense if you choose to participate in other activities outside the team building program planned by the company. It is a requirement that all staff are present and participate in all team building activities for the period.

 

I, _________________________________, hereby acknowledge and agree to the following, as a condition of participating at the 372 Digital “[ACTIVITY NAME]” from [DD/MM/YYYY] until [DD/MM/YYYY]:

 

  1.  My involvement and/or participation in the activity is voluntary, and I am acting under my own free will.
  2. I ACKNOWLEDGE that participation in the activity involves inherent risks, including risk of physical or psychological injury, pain, suffering, illness, disfigurement, temporary or permanent paralysis and or/ death, and I assume all related risks and voluntarily participate in the activity.
  3. I DECLARE that I do not have any medical ailments, physical limitations, or mental disabilities that will affect my ability to participate in this activity.
  4. 372 Digital Pty Ltd undertakes no direct legal or financial responsibility for my personal safety or well being when I am participating in the activity.
  5. I assume the risks, including, but not limited to, those outlined in item 2 of this agreement.
  6. I forever release 372 Digital Pty Ltd from any and all claims and causes of action that I or my representatives now have or may have in the future for personal injury, property damage or wrongful death occurring to me, arising out of participation in this activity.
  7.  I am 100% liable for all medical expenses incurred as a result of any injury or property damage during my participation in this activity.
  8. In the event that any one or more of the provisions of this agreement shall be held to be invalid, illegal, unenforceable or in conflict with the law according to the jurisdiction of the state of ACT, the remaining portions will not be invalidated, and shall remain in full force and effect.
  9. This is a legally binding contract, but it is not meant to pronounce any claims or defences that are legally prohibited.

 

I attest that I have read and understand this document, and agree to all the provisions listed above.
